Shift Type: Weekdays, 4 x 9-hour or 4 x 10-hour day shifts (typical day shift schedule); flexible start times; occasional weekend backup and on-call

Candidate Pay Rate: $53 per hour

Roles & Responsibilities
- Provide compassionate hospice nursing care and symptom management for patients in home, nursing home, group home, or assisted living settings
- Conduct thorough assessments and develop individualized care plans that honor patient goals
- Lead and coordinate the interdisciplinary team for an assigned caseload, ensuring seamless communication and continuity of care
- Educate patients and caregivers so families feel confident and supported in day-to-day care
- Perform timely documentation and care coordination to meet clinical and regulatory expectations
- Serve as a clinical resource for the team and provide occasional weekend backup and on-call support
Qualifications
- Active Registered Nurse (RN) license in Colorado
- Comfortable and experienced with hospice nursing principles and symptom management
- Strong assessment skills and critical thinking to manage complex end-of-life needs
- Experience in patient and caregiver education and leading interdisciplinary care
- Willingness to provide occasional weekend backup and on-call coverage
