Roles & Responsibilities
- Evaluate patients' mobility and functional status to create meaningful therapy plans
- Develop and implement individualized treatment programs that improve independence
- Provide compassionate hands-on care and therapeutic exercise instruction
- Document progress and outcomes clearly and promptly to support care continuity
- Collaborate with nursing and interdisciplinary team members to coordinate patient goals
- Educate patients and families on home programs, fall prevention and safe mobility
- Advocate for patient needs within the skilled nursing environment to optimize recovery
Qualifications
- Licensed PT or DPT (PT/DPT listed as required skills)
- Open to clinicians at all experience levels — any experience accepted, including new grads
- Comfortable working in a Skilled Nursing Facility (SNF) setting
- Strong communication skills and a team-first attitude
Education
- DPT or equivalent Physical Therapy degree (PT/DPT listed as job skills)
Knowledge & Skills
- Skilled patient assessment and goal-setting in post-acute/SNF environments
- Therapeutic exercise, gait training and ADL-focused interventions
- Safe transfer and mobility training techniques
- Clear clinical documentation and progress note writing
- Ability to work collaboratively with interdisciplinary teams
- Time management and caseload organization in a busy SNF
