Shift Type: Monday-Friday, 8:00 AM - 4:00 PM (day); occasional weekends and on-call required

Candidate Pay Rate: $63 per hour

Roles & Responsibilities
- Assess patients and caregivers using the nursing process to create person-centered care plans
- Develop, document and communicate realistic plans of care in line with evidence-based practice and regulations
- Serve as case manager for assigned patients, managing caseloads and visit timing to meet productivity standards
- Complete timely and accurate OASIS and clinical documentation to ensure regulatory compliance
- Implement medical and nursing interventions, including IV therapy and wound VAC care, to maintain high-quality outcomes
- Coordinate interdisciplinary care with physicians, therapists, social work and vendors to support the plan of care
- Educate patients and caregivers about medications, symptom management, discharge instructions and care options
- Monitor infection control practices and ensure high-tech skills are maintained safely across the plan of care
- Collaborate with LPNs and HCA staff, delegating appropriately and supervising care delivery
- Participate in peer review audits, case conferences and quality improvement activities to enhance patient care
- Coordinate safe discharges and transfers, ensuring all required paperwork and communication are complete
- Respond to and report patient incidents in a timely manner and take action to protect patient safety
Qualifications
- Active Registered Nurse (RN) license (Vermont State Board of Nursing requirements referenced)
- Experience in hospice or home health nursing and comfort with home visits
- Proven experience with OASIS documentation and McKesson or similar clinical systems
- Skills in IV therapy, wound VAC management and complex wound care
- Solid case management experience and ability to manage caseloads efficiently
- Strong infection control knowledge and evidence-based practice application
- Excellent communication skills for patient/caregiver education and interdisciplinary coordination
