Roles & Responsibilities

• Manage patient care actively for SEARHC patients, ensuring coordinated, patient-centered outcomes
• Collaborate with medical providers and interdisciplinary team members to set individualized medical goals
• Coordinate and track patient referrals to specialty clinics and surgery, ensuring necessary information is available
• Serve as the primary clinical contact for care coordination and proactively identify and resolve delays in care
• Perform staff nursing duties including assessment, documentation, patient education, and medication administration
• Maintain annual competencies, adhere to infection control policies, and contribute to a safe clinical environment
• Communicate effectively with team members and participate in department improvement efforts
• Participate in special projects as assigned to support comprehensive health services

Qualifications

• Active, full, unrestricted Alaska RN license required at time of submittal
• Basic Life Support (BLS) certification required
• Flu vaccination required
• At least 1 year of case management experience
• Graduate of an accredited school of nursing; Bachelor's degree preferred
• Strong oral and written communication skills and ability to multi-task in a fast-paced clinic
• Team-player attitude with willingness to teach and ask for help when needed

Education

• Graduate of an accredited school of nursing (Associate or Diploma). Bachelor's degree in Nursing preferred
• High school diploma or GED

Knowledge & Skills

• Knowledge of the nursing process and ability to apply theory-based nursing practice
• Customer service principles and strong interpersonal skills
• Safety and infection control principles
• Assessment, anticipation of needs, and data collection skills
• Problem solving and critical thinking to improve patient care processes
• Ability to collaborate in a multidisciplinary team and participate in shared governance
