Roles & Responsibilities

• Evaluate patients on admission and perform thorough, skilled assessments to drive individualized care plans
• Provide skilled treatment sessions that focus on mobility, safety, and functional improvements
• Document admission evaluations, progress notes, treatment records, and discharge summaries accurately and timely
• Coordinate with nursing and interdisciplinary team members to ensure seamless patient care in the Skilled Nursing Facility
• Educate patients and families on home programs and safe discharge plans to promote long-term outcomes
• Participate in quality initiatives and contribute to a positive, family-friendly facility culture

Qualifications

• Licensed Physical Therapist in California or eligible for licensure
• Current BLS/CPR certification
• Experience in a Skilled Nursing Facility or post-acute setting preferred
• Strong clinical documentation skills and comfort with admission, progress, and discharge documentation
• Excellent communication and team collaboration skills
• Ability to pass background checks and required health screenings

Education

• Doctor of Physical Therapy (DPT) preferred or equivalent degree as required for licensure
• Completion of accredited physical therapy program and state licensure

Knowledge & Skills

• Patient evaluation and development of skilled treatment plans
• Proficient clinical documentation including admission evaluations, progress notes, and discharge summaries
• Familiarity with Skilled Nursing Facility workflows and regulatory documentation
• Ability to deliver compassionate, family-centered care
• Time management and prioritization for a busy 45-bed facility
