Roles & Responsibilities
- Manage a panel of SEARHC primary care patients through active case management, improving outcomes and care continuity
- Collaborate with providers and interdisciplinary teams to set individualized medical goals and coordinate care plans
- Coordinate specialty clinic referrals and manage waitlists, ensuring pre-appointment diagnostics and follow-up are completed
- Serve as primary case manager for OB patients: intake, education, outreach, postpartum tracking, and newborn immunizations
- Perform clinical nursing duties in the clinic including assessment, documentation, patient education, and medication/therapy administration
- Lead multidisciplinary case conferences for high-risk or complex patients and provide periodic status reports to providers
- Proactively identify barriers to care, track referral progress, and implement solutions to improve patient access and outcomes
- Support VTC telehealth visits and coordinate Exercise Treadmill Test (ETT) scheduling with local and regional partners
- Maintain infection control standards and annual nursing competencies to keep patients and staff safe
Qualifications
- Active, full, unrestricted Alaska Registered Nurse license required at time of submittal
- Minimum 1 year of case management experience (case management experience must be within the last 2 years)
- Basic Life Support (BLS) certification preferred
- National Case Management certification preferred
- High school diploma or equivalent required; graduate of an accredited school of nursing required (Bachelor's degree preferred)
- Ability to pass fingerprinting requirements (fingerprint cards required; start date coordinated after fingerprints are received)
- Proof of flu vaccination may be required per client policy
Education
- Graduate of an accredited school of nursing (required)
- Bachelor of Science in Nursing (BSN) preferred
- High school diploma or equivalent required
Knowledge & Skills
- Strong knowledge of the nursing process and ability to apply it in a fast-paced clinic environment
- Familiarity with SEARHC services and other tribal health organizations in Alaska
- Proficiency with electronic health records (EHR) and documentation best practices
- Excellent assessment, triage, and patient education skills
- Referral management, care coordination, and multidisciplinary collaboration
- Good communication, conflict resolution, and customer service abilities
- Comfort with telehealth (VTC) technology and virtual visit workflows
- Ability to multi-task, prioritize, and work independently while supporting team-based care
