Roles & Responsibilities

• Manage active primary care patient panels with compassionate, outcomes-focused case management
• Collaborate with providers and interdisciplinary team members to set personalized, achievable medical goals
• Coordinate multidisciplinary case conferences for high‑risk or complex patients
• Perform triage and staff nursing duties including assessment, documentation, patient education, and medication/therapy administration
• Track and manage specialty referrals and waitlists, ensuring pre-appointment diagnostics are complete
• Serve as the primary link between case managers, discharge coordinators, and specialty providers to ensure seamless care transitions
• Proactively identify obstacles to care, report status to providers, and work to resolve delays
• Support telehealth/videoconferencing visits and assist clinical procedures as needed

Qualifications

• Current, full, unrestricted Alaska RN license required at time of submittal
• Minimum 1 year of case management experience (case management experience within the last 2 years required)
• Fingerprint card required (start date dependent on fingerprint clearance timeline)
• Proof of flu vaccination required
• Basic Life Support (BLS) preferred
• National Case Management Certification preferred
• BSN preferred but not required

Education

• Graduate of an accredited school of nursing required
• BSN preferred
• High school diploma or equivalent required

Knowledge & Skills

• Strong patient assessment, triage, and clinical documentation skills
• Experience with care coordination, referral management, and multidisciplinary teamwork
• Familiarity with EHR workflows and triggers for chronic and preventive care
• Knowledge of infection control and safety principles
• Excellent oral and written communication and conflict resolution skills
• Ability to multi-task, work independently in a fast-paced clinic, and support specialty clinics (including OB case management and VTC/telehealth)
