Roles & Responsibilities

• Triaging emergent situations and applying advanced nursing judgment independently
• Monitor and manage acute and chronic medical conditions in coordination with PCPs
• Provide clinic and home-based direct care using individualized care plans
• Administer treatments, procedures and medications as prescribed
• Perform regular home visits including wound care, lab draws and nursing assessments
• Coordinate care with caregivers, interdisciplinary teams and external providers
• Document clinical findings and interventions accurately in the patient record

Qualifications

• Active RN license (Michigan) required
• Associate degree in Nursing required; Bachelor’s degree in Nursing (BSN) preferred
• Minimum of 1 year experience with frail/elderly or long-term care population
• Home care experience and wound care experience preferred
• Current CPR certification from AHA or ability/willingness to obtain it
• Reliable transportation, valid Michigan driver’s license and car insurance
• Must be 18 or older and fully vaccinated (or willing to be) for Influenza, MMR, Varicella, Tdap/Td, COVID-19 and Hepatitis B

Education

• Associate Degree in Nursing (required)
• Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Strong patient assessment and triage skills
• Medication administration and treatment procedure competence
• Care planning and care coordination across settings
• Experience with home visits and home-based safety assessments
• Wound care and basic lab draw experience
• Excellent communication skills with participants, caregivers and clinical teams
• Comfort working independently and managing complex clinical situations
• Familiarity with documentation and electronic medical record workflows
