Roles & Responsibilities
- Provide compassionate, resident-centered nursing care in a long term care setting
- Assess and document resident health status, changes, and care responses
- Develop and implement individualized care plans in collaboration with the care team
- Safely administer medications and treatments according to policy and physician orders
- Monitor and manage chronic conditions common in long term care populations
- Communicate effectively with residents, families, and interdisciplinary team members
- Support quality improvement efforts and maintain accurate clinical records
Qualifications
- Current, active Registered Nurse (RN) license in New York or eligibility for state licensure
- BLS (Basic Life Support) certification required; additional certifications welcomed
- Prior experience in long term care or skilled nursing is highly valued
- Ability to pass facility background checks, drug screening, and health requirements
- Strong interpersonal skills and a compassionate approach to resident care
Education
- Associate Degree in Nursing (ADN) required or Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Geriatric nursing principles and long term care best practices
- Medication administration and documentation accuracy
- Wound care basics and chronic disease management
- Infection control and safety protocols
- Comfort with electronic medical records and charting
- Excellent communication, assessment, and clinical judgment skills
