Roles & Responsibilities

• Provide compassionate, resident-centered nursing care in a long term care setting
• Assess and document resident health status, changes, and care responses
• Develop and implement individualized care plans in collaboration with the care team
• Safely administer medications and treatments according to policy and physician orders
• Monitor and manage chronic conditions common in long term care populations
• Communicate effectively with residents, families, and interdisciplinary team members
• Support quality improvement efforts and maintain accurate clinical records

Qualifications

• Current, active Registered Nurse (RN) license in New York or eligibility for state licensure
• BLS (Basic Life Support) certification required; additional certifications welcomed
• Prior experience in long term care or skilled nursing is highly valued
• Ability to pass facility background checks, drug screening, and health requirements
• Strong interpersonal skills and a compassionate approach to resident care

Education

• Associate Degree in Nursing (ADN) required or Bachelor of Science in Nursing (BSN) preferred

Knowledge & Skills

• Geriatric nursing principles and long term care best practices
• Medication administration and documentation accuracy
• Wound care basics and chronic disease management
• Infection control and safety protocols
• Comfort with electronic medical records and charting
• Excellent communication, assessment, and clinical judgment skills
