Shift Type: Varies; schedule determined by patient needs and assignment (may include day, evening and weekend shifts)
Candidate Pay Rate: $60 per hour

Roles & Responsibilities
- Assess patients using the nursing process to establish individualized plans of care
- Develop and update care plans that reflect patient goals and clinical needs
- Implement skilled nursing interventions in home care, hospice and assisted living settings
- Evaluate patient responses and document outcomes clearly and professionally
- Coordinate with interdisciplinary teams to ensure compassionate, comprehensive care
- Provide patient and family education to support safe care at home and through end of life
- Demonstrate clinical competency while adhering to the Minnesota Nurse Practice Act and facility policies
Qualifications
- Current, active Registered Nurse licensure in Minnesota or eligibility to obtain licensure
- Experience in home health, hospice or assisted living preferred
- BLS certification required; ACLS or hospice-specific certifications a plus
- Strong clinical assessment and care-planning skills
- Comfortable with home visit environments and independent decision making
- Excellent communication, documentation and teamwork skills
- Reliable transportation for community-based visits
Education
- Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred
Knowledge & Skills
- Proficient with the nursing process: assessment, planning, implementation and evaluation
- Patient assessment and focused clinical observations
- Skilled at creating and updating individualized care plans
- Knowledge of hospice principles, symptom management and end-of-life care
- Familiarity with Minnesota Nurse Practice Act and facility policies
- Strong documentation practices and basic electronic medical record navigation
- Compassionate patient and family education and communication
