Roles & Responsibilities

• Assess patient condition in the home setting and develop individualized plans of care that maximize mobility and independence
• Perform standard therapy measurements including ROM, strength and balance testing to track progress
• Deliver a variety of therapeutic modalities to help patients reach functional goals
• Case manage home health patients, including OASIS documentation and coordination of services
• Collaborate with an interdisciplinary team to ensure comprehensive, person-centered care
• Document thoroughly in the EMR to maintain continuity and quality of care

Qualifications

• Vermont PT licensure required
• Home Health experience required; OASIS experience required (candidate will case manage)
• Experience developing individualized plans of care and performing ROM, strength and balance testing
• McKesson or Netsmart EMR experience strongly preferred
• Professional, communicative approach to interdisciplinary collaboration

Knowledge & Skills

• Strong patient assessment and clinical decision-making skills
• OASIS knowledge and home health documentation best practices
• Case management experience in a home health setting
• Proficiency with therapeutic modalities and progressive treatment planning
• Experience with ROM, strength and balance testing
• Comfort collaborating across disciplines to coordinate care
