Roles & Responsibilities

• Perform thorough evaluations to assess patients' physical abilities and design individualized treatment plans
• Deliver evidence-based physical therapy interventions to improve strength, mobility, and functional independence
• Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications
• Collaborate with occupational therapists, nurses, physicians, and the interdisciplinary team to coordinate care
• Monitor patient progress, adjust treatment plans, and document outcomes clearly and promptly
• Ensure care plans meet regulatory standards and best practices in rehabilitation

Qualifications

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
• Valid Physical Therapy license in California (or ability to obtain state licensure)
• Experience in a skilled nursing facility or similar post-acute care setting preferred
• Strong communication, interpersonal, and problem-solving skills
• Ability to work independently and collaboratively within a multidisciplinary team
• Compassionate, patient-focused approach with a commitment to quality care

Education

• Bachelor's, Master's, or Doctoral degree in Physical Therapy (from an accredited program)

Knowledge & Skills

• Proficient in patient assessment and development of individualized treatment plans
• Familiar with therapeutic modalities and adaptive equipment to enhance patient outcomes
• Skilled in documentation, progress notes, and regulatory compliance
• Excellent patient education techniques and safe transfer/mobility training
• Team-oriented mindset with strong collaboration skills across disciplines
• Adaptable and flexible scheduling capabilities for dynamic care needs
