Roles & Responsibilities
- Perform thorough evaluations to assess patients' physical abilities and design individualized treatment plans
- Deliver evidence-based physical therapy interventions to improve strength, mobility, and functional independence
- Educate patients and families on therapeutic exercises, safe movement techniques, and home modifications
- Collaborate with occupational therapists, nurses, physicians, and the interdisciplinary team to coordinate care
- Monitor patient progress, adjust treatment plans, and document outcomes clearly and promptly
- Ensure care plans meet regulatory standards and best practices in rehabilitation
Qualifications
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program
- Valid Physical Therapy license in California (or ability to obtain state licensure)
- Experience in a skilled nursing facility or similar post-acute care setting preferred
- Strong communication, interpersonal, and problem-solving skills
- Ability to work independently and collaboratively within a multidisciplinary team
- Compassionate, patient-focused approach with a commitment to quality care
Education
- Bachelor's, Master's, or Doctoral degree in Physical Therapy (from an accredited program)
Knowledge & Skills
- Proficient in patient assessment and development of individualized treatment plans
- Familiar with therapeutic modalities and adaptive equipment to enhance patient outcomes
- Skilled in documentation, progress notes, and regulatory compliance
- Excellent patient education techniques and safe transfer/mobility training
- Team-oriented mindset with strong collaboration skills across disciplines
- Adaptable and flexible scheduling capabilities for dynamic care needs
