Roles & Responsibilities

• Provide high-quality diabetes self-management education (DSMES) to patients and families, empowering lasting behavior change
• Perform nutritional assessments and develop individualized Medical Nutrition Therapy care plans
• Document all patient interactions accurately in EPIC/EMR with attention to detail
• Train and educate patients on blood glucose monitoring, including continuous glucose monitoring (CGMS), insulin pumps, and injectable medications
• Collaborate closely with a multidisciplinary diabetes management team to coordinate patient-centered care
• Contribute to clinic and program development, including policy/procedure updates and quality improvement initiatives
• Develop patient and staff education materials and serve as a resource for diabetes education across the system
• Participate in peer review and ensure program compliance with ADA requirements

Qualifications

• Bachelor’s degree in Dietetics/Food and Nutrition from a four-year college/university
• Completion of an accredited or approved supervised practice program
• State licensure/certification as a Registered/Licensed Dietitian in Illinois and Wisconsin
• CDCES (Certified Diabetes Care and Education Specialist) certification required
• Prior ADA program experience preferred
• Current CEU/CME maintenance to meet licensure/registration/certification requirements

Education

• Bachelor's degree in Dietetics, Food and Nutrition or related field
• Completion of an accredited supervised practice program (required)

Knowledge & Skills

• Strong expertise in DSMES and Medical Nutrition Therapy
• Skilled in nutritional assessment and individualized care plan development
• Proficient with EPIC and other Electronic Medical Record (EMR) systems
• Experience with Continuous Glucose Monitoring (CGMS), insulin pump training, and injectable medication education
• Ability to create clear, engaging patient and staff education materials
• Basic computer systems knowledge and accurate charge entry skills
• Comfortable participating in quality improvement, peer review, and program development activities
