Roles & Responsibilities

• Provide personalized rehabilitative services and patient evaluations that empower older adults to regain independence
• Develop and implement treatment plans based on thorough patient assessment and best practices
• Document patient progress accurately using Net Health and facility systems to support continuity of care
• Educate residents, families, and staff on therapy goals, expected outcomes, and strategies to support recovery
• Collaborate with interdisciplinary team members to create cohesive, resident-centered care plans
• Foster meaningful relationships with residents to enhance engagement, dignity, and quality of life

Qualifications

• Active Occupational Therapy license in Pennsylvania in good standing
• Degree in Occupational Therapy from an accredited program
• Experience or strong interest in working with the senior population and geriatric care
• Comfortable using Net Health or similar documentation systems
• Excellent communication, teamwork, and compassionate patient care skills

Education

• Degree in Occupational Therapy from an accredited program

Knowledge & Skills

• Skilled in patient assessment and treatment planning for geriatric populations
• Proficient in documentation and using electronic therapy systems such as Net Health
• Strong interpersonal skills for effective patient, family, and staff education
• Knowledge of rehabilitative services and long-term care best practices
• Ability to collaborate within interdisciplinary teams to achieve patient-centered goals
