Roles & Responsibilities

• Evaluate and treat residents in skilled nursing and post-acute settings to restore function and independence
• Develop and implement individualized treatment plans that help residents reach their highest functional potential
• Collaborate with an interdisciplinary team to coordinate care and improve outcomes
• Document clinical progress accurately and timely in accordance with facility and regulatory standards
• Participate in care conferences and drive quality improvement initiatives to enhance resident care

Qualifications

• Active California Physical Therapist license (required)
• Strong clinical assessment and treatment planning skills
• Excellent communication skills and the ability to collaborate with interdisciplinary teams
• Skilled nursing or post-acute care experience preferred
• Dependable, flexible, and team-oriented with a commitment to high-quality, compassionate care
