Roles & Responsibilities

• Evaluate residents’ mobility, balance, strength, and functional needs using evidence-based assessment tools
• Develop individualized treatment plans that aim to improve independence and quality of life
• Implement therapeutic interventions including therapeutic exercise, gait training, and transfer training
• Collaborate with nurses, aides, and interdisciplinary team members to coordinate resident care
• Document progress clearly and timely in the medical record and update care plans as needed
• Educate residents and families on home exercise programs and safe mobility strategies
• Monitor outcomes and adjust treatment plans to meet evolving resident goals

Qualifications

• Licensed Physical Therapist in Texas or able to obtain Texas licensure prior to start
• Current CPR/BLS certification (or willing to obtain)
• Experience working with geriatric, long-term care, or skilled nursing populations preferred
• Strong clinical assessment and documentation skills
• Professional, team-oriented attitude and reliable communication skills

Education

• Doctor of Physical Therapy (DPT) preferred; Master’s in Physical Therapy or equivalent considered

Knowledge & Skills

• Skilled in therapeutic exercise, gait and balance training, and transfer techniques
• Knowledge of geriatric mobility challenges and fall prevention strategies
• Comfortable with electronic medical record documentation and timely charting
• Ability to prioritize caseloads and communicate effectively with interdisciplinary teams
• Strong patient education and family communication skills
