Roles & Responsibilities

• Provide direct patient care and compassionate assistance with activities of daily living
• Perform nursing support tasks following established methods and procedures
• Monitor and document patient vital signs and changes in condition
• Support patient mobility, transfers, and safe positioning
• Assist nursing staff with clinical tasks and unit workflows
• Deliver care across a variety of settings including SNF, ICF, medical-surgical units, and home care
• Communicate clearly with the interdisciplinary care team and patient families
• Adhere to infection control, safety protocols, and facility policies

Qualifications

• Current Certified Nursing Assistant (CNA) certification required
• Must be able to work any and all shifts including nights and weekends
• Comfortable working in SNF, ICF, medical-surgical, and home care environments (preferred)
• Current BLS/CPR certification or willingness to obtain prior to start (preferred)
• Ability to pass background checks and meet facility immunization requirements
• Reliable, flexible, and team-oriented with strong patient advocacy skills

Education

• CNA certification required
• High school diploma or equivalent preferred

Knowledge & Skills

• Strong hands-on patient care skills and compassionate bedside manner
• Proficient in taking and recording vital signs and basic observations
• Skilled in safe patient transfers and mobility assistance
• Good clinical documentation and charting habits
• Excellent communication and teamwork abilities
• Adaptable to rotating shifts and changing care environments
• Awareness of infection control and safety best practices
