Roles & Responsibilities

• Assess patients' functional abilities and create meaningful, goal-driven treatment plans
• Provide individualized interventions to improve independence with activities of daily living
• Deliver therapeutic treatments, adaptive techniques, and recommend equipment when needed
• Collaborate with nursing, physical therapy, speech therapy, and families to support holistic care
• Document accurate, timely progress notes and discharge plans in the facility EHR
• Educate patients and caregivers to promote safety, independence, and long-term wellbeing

Qualifications

• Active Occupational Therapist license in good standing (South Dakota licensure required or ability to obtain prior to start)
• NBCOT certification preferred
• Current BLS/CPR certification (or willingness to obtain)
• Prior experience in long term care, skilled nursing, or similar adult/geriatric settings is a plus
• Ability to pass background check and meet facility immunization requirements
• Strong communication skills and a team-oriented mindset

Education

• Bachelor's or Master's degree in Occupational Therapy (required)

Knowledge & Skills

• Skilled in patient assessment and ADL/IADL training
• Experience with mobility training, transfers, and fall prevention strategies
• Proficient with adaptive equipment selection and home safety recommendations
• Clear documentation skills and familiarity with electronic health record systems
• Ability to collaborate effectively with interdisciplinary teams
• Patient-centered approach and strong clinical reasoning
