Shift Type: Typical daytime, visit-based schedule (approximately 8:00 AM - 6:00 PM), Monday–Saturday with weekends as required; day / rotating (visit-driven) coverage

Candidate Pay Rate: $63 per hour

Roles & Responsibilities
- Conduct thorough home health occupational therapy visits and deliver patient-centered care
- Complete OASIS assessments and documentation accurately and on time (ReCertification, RoC, Routine, Eval, Discharge, F/up, Re-Eval, etc.)
- Manage clinical documentation and ensure charting meets agency and client standards
- Coordinate care with patients, families, physicians, and interdisciplinary teams through clear, professional communication
- Travel safely between patient homes within assigned territory and maintain punctual visit schedules
- Adhere to infection control protocols and provide safe care to COVID+ and PUI patients when required
- Meet weekly productivity expectations when on the Guaranteed Hours plan and track visit points according to program guidelines
Qualifications
- Current CA Occupational Therapy License (required)
- BLS (Basic Life Support) certification
- Valid Driver's License, current Auto Insurance, and Vehicle Registration
- Up-to-date immunizations and screenings: TB (annual), MMR, Varicella, Hepatitis B, Tdap (within 10 years), annual Flu shot, drug screen, and physical
- Home health experience preferred but not required — new grads encouraged to apply
- Comfort working with COVID+ and PUI patients (for Non-Guaranteed option) and willingness to travel outside the service area when needed
Education
- Entry-level or graduate degree in Occupational Therapy (OT) leading to CA OT licensure
Knowledge & Skills
- Strong knowledge of OASIS documentation and home health assessment procedures
- Excellent clinical documentation and charting skills
- Ability to manage time effectively and convert visits into approved hours/points
- Comfortable with independent decision-making in a home setting and adaptable to varied patient needs
- Good communication skills for coordination with interdisciplinary teams, patients, and families
- Familiarity with productivity/per-visit payment models and willingness to meet weekly visit expectations
