Roles & Responsibilities
- Provide skilled physical therapy interventions under the supervision of a PT to help veterans regain function and independence
- Deliver compassionate, patient-centered care tailored to each resident’s goals and medical needs
- Document treatment plans, progress notes, and outcomes accurately and timely
- Collaborate with interdisciplinary team members to coordinate care and optimize resident outcomes
- Educate residents and families on safe mobility strategies, fall prevention, and home exercise programs
- Monitor and report changes in patient status to the supervising PT and clinical team
Qualifications
- Active Physical Therapist Assistant (PTA) license — Montana license preferred or ability to obtain prior to start
- Recent experience in long-term care, skilled nursing, or geriatrics is preferred
- Basic Life Support (BLS/CPR) certification (or willingness to obtain)
- Strong communication skills and a team-first attitude
- Ability to pass facility-required background and health screenings
Education
- Associate degree from an accredited Physical Therapist Assistant program
Knowledge & Skills
- Hands-on experience with therapeutic exercises, gait training, transfers and mobility aids
- Knowledge of documentation standards for skilled nursing and rehabilitation settings
- Skill in individualized treatment planning and progress measurement
- Comfort working with an older adult and veteran population, including comorbidities common in long-term care
- Proficiency in patient safety, fall prevention, and body mechanics
