Roles & Responsibilities

• Provide skilled physical therapy interventions under the supervision of a PT to help veterans regain function and independence
• Deliver compassionate, patient-centered care tailored to each resident’s goals and medical needs
• Document treatment plans, progress notes, and outcomes accurately and timely
• Collaborate with interdisciplinary team members to coordinate care and optimize resident outcomes
• Educate residents and families on safe mobility strategies, fall prevention, and home exercise programs
• Monitor and report changes in patient status to the supervising PT and clinical team

Qualifications

• Active Physical Therapist Assistant (PTA) license — Montana license preferred or ability to obtain prior to start
• Recent experience in long-term care, skilled nursing, or geriatrics is preferred
• Basic Life Support (BLS/CPR) certification (or willingness to obtain)
• Strong communication skills and a team-first attitude
• Ability to pass facility-required background and health screenings

Education

• Associate degree from an accredited Physical Therapist Assistant program

Knowledge & Skills

• Hands-on experience with therapeutic exercises, gait training, transfers and mobility aids
• Knowledge of documentation standards for skilled nursing and rehabilitation settings
• Skill in individualized treatment planning and progress measurement
• Comfort working with an older adult and veteran population, including comorbidities common in long-term care
• Proficiency in patient safety, fall prevention, and body mechanics
