Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents in a long-term care setting
• Administer medications and treatments accurately and document thoroughly
• Monitor resident condition, record vital signs, and report changes to the care team
• Assist with activities of daily living and promote resident dignity and independence
• Collaborate with RNs, therapists, and physicians to develop and follow care plans
• Perform wound care and toileting assistance in line with facility protocols
• Maintain clear, timely charting in the medical record and support accurate shift handoffs
• Support infection control practices and maintain a safe environment for residents

Qualifications

• Current Licensed Practical Nurse (LPN) license — Massachusetts license preferred or ability to obtain
• Basic Life Support (BLS/CPR) certification required or willing to obtain
• Previous experience in long-term care or rehabilitation is a plus
• Ability to pass background checks, TB screening, and required health screenings
• Strong communication skills and a team-first attitude
• Reliable transportation for getting to per-diem shifts

Education

• Graduate of an accredited Practical Nursing program
• Current LPN license (state-specific) and completion of required clinical training

Knowledge & Skills

• Medication administration and documentation best practices
• Basic assessment skills: vital signs, mental status, and skin integrity
• Familiarity with long-term care workflows and care plan implementation
• Comfortable with electronic medical records and accurate charting
• Time management, prioritization, and the ability to work independently when needed
• Compassionate resident-centered care and strong interpersonal skills
