Roles & Responsibilities
- Provide compassionate hands-on nursing care to residents in a long-term care setting
- Administer medications and treatments accurately and document thoroughly
- Monitor resident condition, record vital signs, and report changes to the care team
- Assist with activities of daily living and promote resident dignity and independence
- Collaborate with RNs, therapists, and physicians to develop and follow care plans
- Perform wound care and toileting assistance in line with facility protocols
- Maintain clear, timely charting in the medical record and support accurate shift handoffs
- Support infection control practices and maintain a safe environment for residents
Qualifications
- Current Licensed Practical Nurse (LPN) license — Massachusetts license preferred or ability to obtain
- Basic Life Support (BLS/CPR) certification required or willing to obtain
- Previous experience in long-term care or rehabilitation is a plus
- Ability to pass background checks, TB screening, and required health screenings
- Strong communication skills and a team-first attitude
- Reliable transportation for getting to per-diem shifts
Education
- Graduate of an accredited Practical Nursing program
- Current LPN license (state-specific) and completion of required clinical training
Knowledge & Skills
- Medication administration and documentation best practices
- Basic assessment skills: vital signs, mental status, and skin integrity
- Familiarity with long-term care workflows and care plan implementation
- Comfortable with electronic medical records and accurate charting
- Time management, prioritization, and the ability to work independently when needed
- Compassionate resident-centered care and strong interpersonal skills
