Roles & Responsibilities
- Establish collaborative practice programs with oncologists to optimize inpatient and outpatient chemotherapy and supportive care
- Provide expert drug information and review EHR care plans to align with national evidence-based guidelines
- Round with Oncology Fellows, Nurse Practitioners, and Physicians to recommend medication selection, monitoring, and dose adjustments
- Deliver patient-specific medication education using teach-back for oral oncolytics and IV chemotherapy
- Conduct staff development and educational sessions for pharmacy, nursing, and medical teams
- Review and update oncology care plans in the Electronic Health Record for safety and guideline concordance
- Identify and resolve medication-related problems using clinical data and lab results
- Serve as a preceptor and mentor to pharmacy residents and students to build the next generation of oncology pharmacists
- Participate in Oncology Service Line meetings to advance safety, efficacy, and quality of cancer care
- Promote rational, cost-effective therapy with a focus on cost avoidance and medication optimization
Qualifications
- Doctor of Pharmacy (PharmD) or equivalent
- Licensed Pharmacist (state licensure required)
- Board Certified Oncology Pharmacist (BPS/BCOP) required
- Minimum of 3 years of hospital experience and at least 3 years of clinical or oncology practice/training
- Completion of PGY-1 residency with clinical experience OR PGY-2 oncology residency (or equivalent experience)
- Experience providing oncology clinical pharmacy services, chemotherapy management, and patient education
- Strong interdisciplinary communication and precepting experience
Education
- Doctor of Pharmacy (PharmD)
- PGY-1 residency or equivalent clinical experience (minimum 3 years) OR PGY-2 oncology residency or equivalent (minimum 3 years oncology practice)
Knowledge & Skills
- Expertise in oncology pharmacy, chemotherapy (IV and oral), and supportive care practices
- Clinical patient care and therapeutic monitoring with evidence-based guideline application
- Proficient in Electronic Health Record review and oncology care plan management
- Skilled in medication safety, optimization, and cost avoidance strategies
- Experience with drug information consultation and interdisciplinary rounding
- Ability to teach and precept pharmacy residents and students effectively
