Roles & Responsibilities

• Verify medication orders accurately and efficiently to ensure patient safety
• Dispense medications and oversee medication distribution for inpatient units
• Provide comprehensive clinical pharmacy services across patient populations from pediatrics to geriatrics
• Rotate into ambulatory roles such as anticoagulation clinic or oncology and deliver focused outpatient care
• Collaborate with providers, nurses, and pharmacy technicians to optimize medication therapy
• Ensure compliance with state and federal pharmacy laws and facility policies
• Counsel patients and families on medication use, side effects, and monitoring when appropriate
• Document interventions, clinical decisions, and patient encounters clearly and professionally

Qualifications

• Current Pennsylvania pharmacist license or eligibility for reciprocation within 90 days
• Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred
• Basic Life Support (BLS) certification (meets AHA standards) and renewal every two years if working in anticoagulation clinic
• Recent hospital experience preferred
• Pharmacy residency and advanced practice certifications are a plus
• Strong communication skills and ability to work with interdisciplinary teams

Education

• Bachelor of Science in Pharmacy (required)
• Doctor of Pharmacy (PharmD) (preferred)

Knowledge & Skills

• Medication order verification and safe medication dispensing
• Clinical pharmacy practices across inpatient and ambulatory settings
• Knowledge of Pennsylvania state and federal pharmacy laws and regulations
• Patient-centered care and counseling techniques
• Experience with anticoagulation management and outpatient clinic workflows (preferred)
• Team collaboration, problem solving, and attention to detail
