Roles & Responsibilities

• Assess clients across inpatient, outpatient, and emergency settings to determine needs and risks
• Design and manage individualized service plans that promote self-sufficiency and measurable outcomes
• Coordinate care and resources with community social services, mental health, and substance abuse providers
• Perform discharge planning and utilization review to ensure appropriate, efficient care transitions
• Evaluate ED patients for medical necessity and collaborate with clinicians on admission decisions
• Monitor patient progress, respond to emerging needs, and adjust plans in partnership with the care team
• Advocate for patients and facilitate referrals to specialty providers when services are not available onsite
• Participate actively in multidisciplinary team meetings to align care goals and utilization objectives

Qualifications

• Master's degree in Social Work (MSW)
• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) — full, unrestricted license in any U.S. state, territory, or Puerto Rico
• Current Basic Life Support (BLS) certification by the American Heart Association
• Minimum of 3 years of experience in a clinical care setting
• Experience with discharge planning, case management, or utilization review processes
• Familiarity with electronic health record systems

• Utilization review and medical necessity evaluation
• Comfort working in emergency department evaluations and fast-paced clinical settings
• Cultural competence serving Native American communities (Navajo, Hopi, Southern Paiute)
• Strong interdisciplinary communication and collaboration skills
• Proficiency with electronic health record (EHR) documentation
