Roles & Responsibilities

• Assess patients in their homes to determine functional status and therapy needs
• Develop individualized treatment plans that promote mobility, safety, and independence
• Deliver skilled physical therapy interventions during in-home visits
• Document thoroughly and accurately using the HCHB system after each visit
• Educate patients and caregivers on home exercise programs and fall prevention
• Coordinate care with nurses, home health aides, and other members of the care team
• Manage your schedule and travel efficiently to ensure timely patient visits
• Contribute to a patient-centered culture that prioritizes dignity and outcomes

Qualifications

• Minimum 2 years of home health experience (therapy positions require home health experience)
• Proven experience documenting in HCHB (Homecare HomeBase)
• Comfort and experience performing in-home patient visits; reliable vehicle required
• Ability to work Monday through Friday daytime schedule and manage a ~5-6 visit caseload

Knowledge & Skills

• Strong clinical assessment and treatment planning skills for adult and geriatric populations
• Knowledge of home safety, mobility training, transfer techniques, and fall prevention
• Proficient documentation skills in HCHB or similar home health EHR systems
• Excellent communication and caregiver education abilities
• Strong time management and route planning for efficient patient visits
• Ability to collaborate with interdisciplinary home health teams
