Roles & Responsibilities

• Manage patient care proactively by collaborating with providers and interdisciplinary teams to set and achieve individualized medical goals
• Coordinate referrals to specialty clinics and surgery, review referral records, and track outcomes to ensure timely care
• Serve as the primary clinical contact for patients, identifying delays or obstacles and driving solutions to improve care pathways
• Perform hands-on nursing duties including patient assessment, documentation, education, and medication administration
• Maintain infection control and safety standards, ensuring a clean, well-stocked, and safe work environment
• Communicate clearly across teams, build rapport with colleagues, and participate in clinic improvement efforts
• Contribute to special projects and process improvements that enhance patient care and clinic operations

Qualifications

• Active, full, unrestricted RN license in the State of Alaska required at time of submittal
• Basic Life Support (BLS) certification required
• Flu vaccination required prior to start
• Minimum 1 year of case management experience
• BSN preferred (but graduates of accredited nursing programs are encouraged to apply)
• Willingness to complete fingerprint/background checks (traveler start not less than 3 weeks after fingerprint cards received at SEARHC in Juneau)
• Strong critical thinking, communication, and teamwork skills

Education

• Graduate of an accredited school of nursing (Bachelor’s degree preferred)
• High school diploma or GED

Knowledge & Skills

• Solid understanding of the nursing process and evidence-based practice
• Skilled in patient assessment, care coordination, and clinical documentation
• Medication administration and infection control best practices
• Referral coordination and tracking outcomes of care
• Patient education, problem solving, and effective oral/written communication
• Ability to multitask independently in a fast-paced clinical environment
