Roles & Responsibilities
- Provide compassionate, resident-centered care for long-term care patients, ensuring dignity and comfort every shift
- Administer medications and treatments accurately and document care in PointClickCare (PCC)
- Perform thorough resident assessments and report changes promptly to the interdisciplinary team
- Float between two floors as needed to support staffing and continuity of care
- Take on charge nurse responsibilities for your unit when assigned, coordinating daily operations and staff
- Maintain clear, timely clinical documentation and handoffs to support safe, high-quality care
- Support admissions, discharges and transitions with thoughtful communication and attention to detail
- Promote infection control, safety protocols and best practices to protect residents and staff
Qualifications
- Current LPN license (appropriate state licensure required)
- Minimum 2 years recent experience working in a long term care unit
- Familiarity with PointClickCare (PCC) or similar electronic health record systems
- Comfort and experience floating between units/floors
- Willingness and ability to work night shifts
- Strong communication skills and ability to collaborate with a multidisciplinary team
- Previous charge nurse experience or demonstrated leadership on a unit is a plus
Education
- Completion of an accredited practical/vocational nursing program and current LPN licensure

- Long term care best practices and resident-focused care
- Medication administration and nursing assessments
- PointClickCare (PCC) documentation and EHR workflows
- Unit leadership and charge nurse duties
- Infection control, wound care basics and safety protocols
