Roles & Responsibilities
- Provide compassionate direct patient care to residents, promoting dignity and comfort
- Administer medications and treatments safely and according to policy
- Monitor and document vital signs and changes in patient condition
- Collaborate with the interdisciplinary team to create and follow individualized care plans
- Educate patients and families about care, medications and follow up
- Assist with admissions, transfers and discharges to ensure smooth transitions
- Maintain accurate clinical documentation in the facility EMR and uphold regulatory standards
Qualifications
- Active Licensed Practical Nurse (LPN) license required
- Current BLS/CPR certification required or obtained upon hire
- Comfortable working in long term care and skilled nursing settings
- Ability to pass background check and drug screening as required
- Strong communication skills and a team oriented mindset
Education
- Graduate of an accredited Practical Nursing program with LPN diploma or equivalent
Knowledge & Skills
- Proven skills in medication administration and basic nursing procedures
- Solid patient assessment and vital sign monitoring
- Competent charting and documentation in electronic medical records
- Basic wound care and infection control practices
- Good time management and prioritization in a busy clinical environment
- Empathy, strong communication, and collaboration with care teams and families
