Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents in a skilled nursing and rehab setting
• Perform thorough assessments and communicate changes in patient condition promptly
• Administer medications and treatments safely and according to policy
• Collaborate with the interdisciplinary team to develop and implement individualized care plans
• Document care accurately and timely in the medical record
• Support residents with activities of daily living while promoting dignity and independence
• Mentor and support CNAs and other team members to maintain a positive care environment

Qualifications

• Current Licensed Practical Nurse (LPN) license; active and in good standing
• Previous experience in long-term care, skilled nursing or rehab preferred
• BLS (Basic Life Support) certification or willingness to obtain
• Up-to-date immunizations and ability to pass required background checks
• Strong communication skills and a patient-centered approach

Education

• Completion of an accredited LPN program
• High school diploma or GED

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Skilled in wound care basics and recognizing changes in skin integrity
• Comfortable performing resident assessments and reporting concerns
• Familiarity with electronic medical records or willingness to learn
• Ability to prioritize tasks in a fast-paced clinical setting
• Excellent teamwork and interpersonal skills
