Candidate Pay Rate: $46 per hour

Roles & Responsibilities
- Provide compassionate, high-quality nursing care to residents in a skilled nursing and rehab setting
- Perform thorough assessments and communicate changes in patient condition promptly
- Administer medications and treatments safely and according to policy
- Collaborate with the interdisciplinary team to develop and implement individualized care plans
- Document care accurately and timely in the medical record
- Support residents with activities of daily living while promoting dignity and independence
- Mentor and support CNAs and other team members to maintain a positive care environment
Qualifications
- Current Licensed Practical Nurse (LPN) license; active and in good standing
- Previous experience in long-term care, skilled nursing or rehab preferred
- BLS (Basic Life Support) certification or willingness to obtain
- Up-to-date immunizations and ability to pass required background checks
- Strong communication skills and a patient-centered approach
Education
- Completion of an accredited LPN program
- High school diploma or GED
Knowledge & Skills
- Solid knowledge of medication administration and documentation best practices
- Skilled in wound care basics and recognizing changes in skin integrity
- Comfortable performing resident assessments and reporting concerns
- Familiarity with electronic medical records or willingness to learn
- Ability to prioritize tasks in a fast-paced clinical setting
- Excellent teamwork and interpersonal skills
