Roles & Responsibilities
- Provide compassionate direct care to residents, prioritizing safety and dignity
- Administer medications and treatments accurately and document thoroughly
- Monitor and report changes in patient condition to the care team
- Assist with activities of daily living including bathing, dressing, and mobility
- Collaborate with nurses, therapists, and restorative staff to support rehab goals
- Maintain clear, timely charting in the facility’s documentation system
- Support infection control and facility safety protocols on every shift
Qualifications
- Current Licensed Practical Nurse (LPN) license—Massachusetts license preferred or ability to obtain
- CPR/BLS certification required or must obtain upon hire
- Previous experience in long-term care, rehab, or nursing facility preferred
- Strong clinical judgment, compassion, and effective communication skills
- Ability to work 12-hour day shifts and lift/assist residents as needed
Education
- Completion of an accredited LPN program or equivalent
- High school diploma or GED
Knowledge & Skills
- Medication administration and safe documentation practices
- Wound care basics and dressing changes
- Familiarity with electronic medical records and charting
- Knowledge of infection prevention and resident safety standards
- Excellent interpersonal skills for working with residents and families
- Team-oriented approach and ability to follow care plans
