Roles & Responsibilities

• Provide compassionate, direct nursing care to residents as an LPN
• Administer medications and treatments accurately and on schedule
• Monitor and record vital signs and report changes in condition
• Collaborate with RNs and interdisciplinary team to support care plans
• Maintain clear, timely documentation in the patient record
• Support residents with activities of daily living and comfort measures
• Follow infection control practices and facility policy to ensure safety

Qualifications

• Current LPN license valid in Pennsylvania
• Basic Life Support certification required, other certifications welcomed
• Comfortable working in long term care or skilled nursing settings
• Strong communication skills and ability to work well on a team
• Reliable, professional, and committed to quality resident care
• Able to pass facility background checks and health screenings

Education

• Graduate of an accredited Practical Nursing program
• LPN/LVN diploma or certificate required

Knowledge & Skills

• Medication administration and documentation best practices
• Clinical assessment skills including vitals and condition monitoring
• Familiarity with EMR charting and electronic documentation
• Wound care basics and safe infection control practices
• Time management and prioritization during busy evening shifts
• Strong interpersonal skills for resident- and family-centered care
