Roles & Responsibilities
- Provide compassionate, hands-on nursing care to residents throughout the shift
- Administer medications and treatments accurately and document per policy
- Assess residents, record vital signs, and report changes to the care team
- Assist with ADLs and promote resident independence and dignity
- Collaborate with RNs, therapists, and support staff to implement care plans
- Maintain clear, timely documentation in the medical record
- Support a positive environment by coaching and mentoring CNA staff when needed
- Follow infection control and safety protocols to protect residents and staff
Qualifications
- Licensed Practical Nurse (LPN) license valid in Pennsylvania
- Current BLS/CPR certification or willingness to obtain upon hire
- Previous experience in long term care or skilled nursing preferred
- Strong clinical assessment and medication administration skills
- Reliable, team-oriented attitude and good communication skills
Education
- Practical Nursing diploma or certificate from an accredited program
Knowledge & Skills
- Medication administration and accurate charting
- Resident assessment and care planning basics
- Familiarity with infection control and safety procedures
- Comfortable with electronic medical records and documentation
- Excellent interpersonal skills for working with residents and families
