Roles & Responsibilities
- Provide safe, compassionate nursing care to residents as a licensed LPN
- Administer medications and treatments accurately and document per facility policy
- Monitor resident condition, report changes, and collaborate with the care team
- Perform thorough and timely charting in the electronic health record
- Assist with admissions, transfers and discharge activities as needed
- Support resident dignity and independence through person-centered care
- Communicate clearly with families and interdisciplinary staff to coordinate care
- Participate in quality improvement and infection prevention practices
Qualifications
- Current Licensed Practical Nurse (LPN) license (PA licensure preferred/required)
- Basic Life Support (BLS) certification required or obtained upon hire
- Current immunizations and ability to pass required health screenings
- Clean background check and ability to meet facility onboarding requirements
- Previous experience in long-term care or skilled nursing helpful but not required
Education
- Graduate of an accredited LPN / Practical Nursing program
- High school diploma or GED
Knowledge & Skills
- Strong medication administration and IV/SC/IM injection knowledge where applicable
- Skilled in resident assessment, vital signs and wound care basics
- Comfortable with charting and documentation in electronic health records
- Excellent communication and teamwork skills with interdisciplinary staff
- Ability to prioritize tasks in a fast-paced care environment with attention to detail
