Shift Type: Days to be confirmed with facility, 8:00 AM - 8:00 PM (Day shift)
Hours per Week: 36
Candidate Pay Rate: $42 per hour

Roles & Responsibilities
- Provide compassionate, patient-centered care on the rehab and nursing floor
- Administer medications and treatments accurately and on schedule
- Monitor and document vital signs, changes in condition and care interventions
- Perform wound care and assist with restorative nursing activities
- Collaborate with nurses, therapists and physicians to implement individualized care plans
- Educate patients and families about care plans, medications and discharge goals
- Maintain clear, timely documentation in the facility charting system
Qualifications
- Current LPN license in Massachusetts (required)
- Completion of an accredited practical nursing program
- Basic Life Support (BLS/CPR) certification or willingness to obtain prior to start
- Previous experience in long-term care, rehab or skilled nursing preferred
- Strong clinical judgment, communication skills and teamwork mindset
Education
- Graduate of an accredited practical nursing program; LPN license required
Knowledge & Skills
- Proficient with medication administration and patient assessment
- Familiarity with wound care and restorative nursing techniques
- Comfortable with electronic medical records and accurate documentation
- Excellent interpersonal skills for working with residents and families
- Ability to prioritize, multitask and adapt in a fast-paced setting
