Roles & Responsibilities

• Provide compassionate, patient-centered care on the rehab and nursing floor
• Administer medications and treatments accurately and on schedule
• Monitor and document vital signs, changes in condition and care interventions
• Perform wound care and assist with restorative nursing activities
• Collaborate with nurses, therapists and physicians to implement individualized care plans
• Educate patients and families about care plans, medications and discharge goals
• Maintain clear, timely documentation in the facility charting system

Qualifications

• Current LPN license in Massachusetts (required)
• Completion of an accredited practical nursing program
• Basic Life Support (BLS/CPR) certification or willingness to obtain prior to start
• Previous experience in long-term care, rehab or skilled nursing preferred
• Strong clinical judgment, communication skills and teamwork mindset

Education

• Graduate of an accredited practical nursing program; LPN license required

Knowledge & Skills

• Proficient with medication administration and patient assessment
• Familiarity with wound care and restorative nursing techniques
• Comfortable with electronic medical records and accurate documentation
• Excellent interpersonal skills for working with residents and families
• Ability to prioritize, multitask and adapt in a fast-paced setting
