Roles & Responsibilities

• Provide compassionate direct patient care consistent with LPN scope of practice
• Administer medications and document administration accurately and timely
• Perform routine assessments, monitor vital signs, and report changes to the RN/MD
• Collaborate with the interdisciplinary care team to support individualized care plans
• Maintain clear, concise charting in the electronic medical record
• Support patient admissions, discharges, and safe transitions of care
• Apply infection control and safety protocols to protect patients and staff
• Coach patients and families with clear education to promote recovery and independence

Qualifications

• Active Licensed Practical Nurse (LPN) license for Pennsylvania or eligible to obtain prior to start
• Current BLS/CPR certification (required)
• Previous experience in long-term care, skilled nursing, or acute care preferred
• Ability to pass background checks and meet facility onboarding requirements
• Strong communication, organizational, and teamwork skills

Education

• Graduate of an accredited Practical Nursing or Vocational Nursing program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and wound care
• Familiarity with electronic medical records and charting
• Infection prevention and standard precautions
• Time management in a fast-paced clinical environment
• Patient- and family-centered communication
