Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and monitor responses accurately and safely
• Perform thorough patient assessments and document findings clearly
• Collaborate with the care team to implement individualized care plans
• Assist with procedures and activities of daily living while promoting dignity
• Communicate effectively with patients, families, and interdisciplinary staff

Qualifications

• Valid LPN license for Pennsylvania (or eligibility to obtain state licensure)
• Current BLS/CPR certification (or ability to obtain upon hire)
• Previous LPN clinical experience preferred
• Ability to pass facility background checks and required screenings
• Strong interpersonal skills and patient-centered mindset

Education

• Practical Nursing diploma or equivalent (LPN program graduate)

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and vital signs monitoring
• Wound care fundamentals and infection control principles
• Charting in electronic medical records and clear documentation
• Team communication and the ability to follow care plans reliably
