Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in accordance with facility policies
• Administer medications and treatments accurately and document them in the chart
• Monitor patient conditions, report changes promptly, and escalate concerns to the care team
• Perform skilled nursing procedures and assist with activities of daily living as needed
• Collaborate with interdisciplinary team members to create and follow care plans
• Educate patients and families about care plans, medications, and follow-up needs
• Maintain accurate, timely documentation in the electronic health record and support quality initiatives

Qualifications

• Current LPN or RN license in good standing (Delaware license or accepted compact licensure where applicable)
• Basic Life Support (BLS/CPR) certification required
• Recent clinical experience in long-term care, skilled nursing, or similar settings preferred
• Strong communication skills and ability to work collaboratively on a multidisciplinary team
• Current immunizations and ability to complete required background checks and screenings

Education

• LPN: Graduation from an accredited practical nursing program (diploma or certificate)
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) as applicable

Knowledge & Skills

• Clinical assessment and vital signs monitoring with keen attention to change
• Medication administration and safe documentation practices
• Skilled wound care, infection prevention, and ADL support
• Comfortable using electronic health records and completing timely charting
• Patient- and family-centered education and communication
• Time management and prioritization during busy night shifts
