Shift Type: Varies by facility schedule, 2:45 PM - 11:15 PM (Evening / Swing shift)

Candidate Pay Rate: $36 per hour

Roles & Responsibilities
- Administer medications and treatments accurately and safely
- Perform focused patient assessments and report changes promptly
- Document patient care and progress clearly in the EMR
- Support patient comfort, dignity and daily living activities
- Collaborate with nurses, providers and the care team for safe care transitions
- Assist with wound care, vital signs, and routine nursing procedures
- Participate in unit huddles and contribute to quality improvement efforts
Qualifications
- Active Licensed Practical Nurse (LPN) license for practice in Delaware
- BLS (Basic Life Support) certification required; other certifications a plus
- Current immunizations and ability to pass background / health screenings
- Previous long-term care or acute care nursing experience preferred
- Strong communication skills and a team-oriented attitude
Education
- Graduate of an accredited LPN program; high school diploma or equivalent required
Knowledge & Skills
- Medication administration and documentation best practices
- Clinical assessment and monitoring of patient status
- Basic wound care and infection control principles
- Familiarity with electronic medical records (EMR) workflows
- Time management, prioritization, and patient-centered communication
