Roles & Responsibilities
- Provide compassionate, patient-centered nursing care to residents in accordance with care plans
- Administer medications and treatments accurately and document per facility policy
- Assess patient conditions, report changes promptly, and collaborate with the interdisciplinary team
- Perform wound care, vital signs monitoring, and clinical procedures as appropriate
- Educate patients and families on care plans and promote a supportive environment
- Maintain accurate and timely documentation in the patient record
Qualifications
- Active LPN or RN license in good standing (New Hampshire licensure or ability to obtain state license prior to start)
- Current BLS (Basic Life Support) certification; ACLS or other certifications as required by facility
- Previous experience in long-term care or skilled nursing preferred
- Reliable, professional, and team-oriented with strong communication skills
- Ability to pass facility-required background checks and health screenings
Education
- Graduate of an accredited LPN or RN nursing program; ADN/BSN for RNs preferred
- High school diploma or equivalent required
Knowledge & Skills
- Medication administration and medication reconciliation
- Clinical assessment and documentation skills
- Familiarity with infection control standards and patient safety practices
- Ability to work with electronic medical records and complete charting accurately
- Strong interpersonal skills for working with residents, families, and care teams
