Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents in accordance with care plans
• Administer medications and treatments accurately and document per facility policy
• Assess patient conditions, report changes promptly, and collaborate with the interdisciplinary team
• Perform wound care, vital signs monitoring, and clinical procedures as appropriate
• Educate patients and families on care plans and promote a supportive environment
• Maintain accurate and timely documentation in the patient record

Qualifications

• Active LPN or RN license in good standing (New Hampshire licensure or ability to obtain state license prior to start)
• Current BLS (Basic Life Support) certification; ACLS or other certifications as required by facility
• Previous experience in long-term care or skilled nursing preferred
• Reliable, professional, and team-oriented with strong communication skills
• Ability to pass facility-required background checks and health screenings

Education

• Graduate of an accredited LPN or RN nursing program; ADN/BSN for RNs preferred
• High school diploma or equivalent required

Knowledge & Skills

• Medication administration and medication reconciliation
• Clinical assessment and documentation skills
• Familiarity with infection control standards and patient safety practices
• Ability to work with electronic medical records and complete charting accurately
• Strong interpersonal skills for working with residents, families, and care teams
