Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents according to care plans
• Administer medications and treatments safely and accurately
• Monitor and document patient status, reporting changes to the care team
• Assist with activities of daily living to promote independence and dignity
• Collaborate with interdisciplinary staff to support positive patient outcomes
• Maintain accurate, timely charting and follow facility protocols

Qualifications

• Current LPN license in good standing (national/state licensure required)
• Minimum clinical experience preferred; confident in med administration and resident care
• CPR/BLS certification (or willing to obtain prior to start)
• Strong communication skills and ability to work as part of a team
• Ability to pass required background checks and health screenings

Education

• Diploma or certificate in Practical Nursing (LPN program) or equivalent

Knowledge & Skills

• Solid knowledge of medication administration and documentation
• Familiarity with long-term care and skilled nursing workflows
• Patient assessment and basic wound care skills
• Time management and the ability to prioritize care tasks
• Comfortable using electronic health records for charting
