Roles & Responsibilities

• Provide compassionate direct patient care to residents in the facility
• Administer medications and treatments accurately and safely
• Monitor patient condition and report changes to the interdisciplinary team
• Document care and interventions clearly in the patient record
• Collaborate with nurses and other care team members to create effective care plans
• Educate patients and families on care, medications and discharge instructions
• Maintain infection control and safety standards at all times

Qualifications

• Active LPN license (able to practice in Pennsylvania or obtain Pennsylvania licensure)
• Current BLS/CPR certification or willingness to obtain prior to start
• Valid clinical experience as an LPN; travel or contract experience is a plus
• Ability to pass background check and required health screenings
• Strong communication skills and a patient-centered attitude

Education

• Graduate of an accredited Licensed Practical Nurse (LPN) program; required

Knowledge & Skills

• Medication administration and basic clinical assessments
• Wound care and routine treatments
• Ability to prioritize tasks in a fast-paced facility setting
• Comfortable with electronic charting and documentation
• Team collaboration and clear communication with patients and families
