Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents with dignity and respect
• Administer medications and treatments accurately and document thoroughly
• Monitor patient conditions, report changes, and collaborate with the care team
• Assist with ADLs and promote mobility and independence whenever possible
• Support care plan implementation and participate in interdisciplinary care discussions
• Maintain a safe, clean environment and follow infection prevention best practices

Qualifications

• Valid LPN license (practical nurse) — ready to practice in Pennsylvania or eligible to obtain one
• Current BLS/CPR certification or willingness to obtain upon hire
• Prior experience in long-term care or similar settings is a plus
• Strong communication skills and the ability to work well on night shift teams
• Up-to-date immunizations and the ability to pass required background checks

Education

• Graduate of an accredited Practical Nursing program; high school diploma or equivalent required

Knowledge & Skills

• Medication administration and documentation practices
• Basic wound care and infection control procedures
• Patient assessment and vital signs monitoring
• Electronic charting and basic computer literacy
• Teamwork, time management, and resident-centered care approach
