Roles & Responsibilities

• Provide compassionate, high-quality nursing care to residents as a Licensed Practical Nurse
• Administer medications and document treatments accurately and timely
• Perform routine assessments, monitor patient status, and report changes to the care team
• Collaborate with RNs, CNAs, and interdisciplinary staff to create and execute care plans
• Maintain accurate medical records and support EMR/charting workflows
• Support resident comfort and dignity through person-centered care and communication
• Assist with admissions, discharges, and transitions to ensure continuity of care

Qualifications

• Current Licensed Practical Nurse (LPN) license (state required) — Pennsylvania license required for this assignment
• Basic Life Support (BLS/CPR) certification preferred or able to obtain
• Ability to pass background check and drug screening per facility policy
• Comfortable working in long-term care / skilled nursing environments
• Strong communication skills and a team-oriented mindset

Education

• Graduate of an accredited Practical Nursing program

Knowledge & Skills

• Solid knowledge of medication administration and documentation best practices
• Familiarity with common long-term care conditions and interventions
• Comfort using electronic medical records and basic charting systems
• Good clinical assessment skills and the ability to prioritize tasks
• Strong interpersonal skills for patient, family, and team interactions
