Roles & Responsibilities
- Administer medications and treatments accurately and safely
- Monitor and document patient vital signs and condition changes
- Provide hands-on personal care and support for activities of daily living
- Collaborate with RNs and the care team to implement care plans
- Maintain clear, timely documentation in the patient record
- Support patient and family education with compassion and clarity
- Contribute to a positive, person-centered environment at the facility
Qualifications
- Active Licensed Practical Nurse (LPN) license in Massachusetts or eligible for state licensure
- Current BLS/CPR certification or willingness to obtain upon hire
- Comfort working in a long-term care or skilled nursing setting
- Strong communication skills and a team-first attitude
- Professional, compassionate approach to resident care
Education
- Practical Nursing diploma or equivalent and successful completion of an approved LPN program
Knowledge & Skills
- Medication administration and accurate charting
- Basic wound care and infection control practices
- Vital sign assessment and clinical observation
- Familiarity with electronic medical records and documentation standards
- Ability to prioritize tasks and work effectively under supervision
- Good interpersonal skills for resident and family interactions
