Roles & Responsibilities
- Provide compassionate direct nursing care to residents in a skilled nursing setting
- Administer medications and treatments accurately and document per facility policy
- Monitor resident condition, report changes promptly, and escalate concerns to the care team
- Collaborate with nurses, therapists and providers to develop and follow individualized care plans
- Assist residents with activities of daily living and support mobility and safety
- Maintain clear, timely clinical documentation and ensure compliance with facility protocols
- Follow infection control standards and promote a clean, safe environment
- Support resident emotional well-being by building rapport and encouraging participation in activities
Qualifications
- Valid Massachusetts Licensed Practical Nurse (LPN) license required
- Current BLS/CPR certification required or obtained upon hire
- Previous experience in skilled nursing, long-term care or rehabilitation preferred
- Comfortable working 12-hour day shifts and handling the physical demands of resident care
- Up-to-date immunizations and ability to pass facility background and health screenings
Education
- Graduate of an accredited Practical Nursing program
- High school diploma or equivalent
Knowledge & Skills
- Strong medication administration and clinical assessment skills
- Skilled in charting and documentation; comfortable with electronic medical records
- Good communication and teamwork skills to coordinate resident care
- Ability to prioritize tasks and respond quickly in changing clinical situations
- Knowledge of infection control practices and safety protocols
