Roles & Responsibilities
- Provide compassionate direct patient care, ensuring dignity and comfort
- Administer medications and treatments accurately and document promptly
- Monitor resident condition, report changes and collaborate with the care team
- Assist with activities of daily living and promote resident independence
- Communicate clearly with families and interdisciplinary staff to coordinate care
- Support a safe, clean environment following infection control best practices
Qualifications
- Current Licensed Practical Nurse (LPN) credential in good standing
- Prior experience in a long-term care, rehab or skilled nursing setting preferred
- Current CPR/BLS certification or willingness to obtain upon hire
- Strong interpersonal skills and ability to work as part of a multidisciplinary team
- Comfortable working 12-hour day shifts and handling routine clinical tasks
Education
- Completion of an accredited Practical Nursing program
- High school diploma or GED
Knowledge & Skills
- Medication administration and accurate charting
- Basic wound care and monitoring of vital signs
- Familiarity with electronic health records and documentation best practices
- Patient-centered communication and family education
- Time management, prioritization and teamwork in a busy clinical setting
