Roles & Responsibilities

• Provide compassionate, patient-centered nursing care within your scope as an LPN
• Administer medications and treatments accurately and in a timely manner
• Perform thorough patient assessments and report changes to the care team
• Document care and observations clearly in the facility EHR
• Collaborate with RNs, providers and support staff to coordinate patient care
• Support safe infection control practices and maintain a clean care environment
• Assist with admissions, discharges and smooth transitions of care

Qualifications

• Active LPN license (Delaware preferred) — facility may consider RN candidates as well
• Current basic life support (BLS) preferred where required by facility policy
• Valid professional licensure in good standing with no restrictions
• Ability to pass background and drug screen as required by the facility
• Strong communication skills and a patient-first attitude

Education

• Completion of an accredited practical nursing program (LPN)
• High school diploma or equivalent required

Knowledge & Skills

• Solid knowledge of medication administration and documentation standards
• Familiarity with basic nursing assessments and vital sign monitoring
• Comfort using electronic health records for charting and orders
• Good clinical judgment, time management and teamwork skills
• Infection control, wound care basics and patient safety practices
