Roles & Responsibilities

• Provide compassionate, patient-centered nursing care to residents and patients
• Administer medications and treatments accurately and on schedule
• Assess and monitor patient conditions, reporting changes promptly
• Document care, observations, and interventions clearly and professionally
• Collaborate with nurses, therapists, and multidisciplinary team members to support recovery goals
• Educate patients and families about care plans and wellness strategies
• Follow facility policies, safety procedures, and infection control practices to protect patients and staff

Qualifications

• Active Licensed Practical Nurse (LPN) license (Pennsylvania) required
• Comfortable working in long-term care and rehab settings; prior LTC experience preferred
• Current basic life support (BLS) certification preferred or willing to obtain
• Strong clinical assessment, medication administration, and documentation skills
• Professional, compassionate communication with patients, families, and team members

Education

• Graduate of an accredited practical nursing program (LPN diploma or equivalent)

Knowledge & Skills

• Solid knowledge of practical nursing principles and standards of care
• Skilled in medication administration and basic wound care
• Ability to perform timely patient assessments and recognize changes
• Good charting practices and familiarity with electronic documentation
• Strong teamwork, time management, and patient advocacy skills
