Roles & Responsibilities
- Provide compassionate hands-on nursing care for residents in a skilled nursing setting
- Administer medications and treatments accurately and document thoroughly
- Monitor resident condition and communicate changes promptly to the care team
- Assist with daily living activities while encouraging resident independence
- Collaborate with nurses, therapists and physicians to deliver individualized care plans
- Educate residents and families about care, safety and follow-up needs
- Maintain professional standards, safety protocols and infection control practices
Qualifications
- Current Licensed Practical Nurse (LPN) license valid in Pennsylvania
- Current Basic Life Support (BLS) or CPR certification
- Up-to-date immunizations and ability to pass background and drug screening
- Comfortable working in a long-term care or skilled nursing environment
- Strong communication, teamwork and time management skills
- Flexible, reliable and committed to resident-centered care
Education
- Completion of an accredited Practical Nursing program and active LPN licensure
Knowledge & Skills
- Medication administration and accurate clinical documentation
- Basic wound care, vital signs and resident assessment
- Familiarity with care plans, ADLs and restorative nursing techniques
- Infection control best practices and safe patient handling
- Ability to prioritize tasks in a fast-paced clinical setting
- Compassionate bedside manner and strong interpersonal skills
