Roles & Responsibilities

• Provide compassionate, patient-centered care as a Licensed Practical Nurse in the facility
• Administer medications and treatments accurately and document care thoroughly
• Perform regular patient assessments and report changes to the care team promptly
• Collaborate with nurses and interdisciplinary staff to deliver safe, coordinated care
• Support patient comfort, dignity, and daily living needs with professionalism
• Help maintain accurate clinical records and comply with facility policies and regulations

Qualifications

• Active LPN (Licensed Practical Nurse) license in Pennsylvania (required)
• Current BLS/CPR certification (preferred) or willingness to obtain promptly
• Comfortable providing bedside nursing care in a long-term/rehab setting
• Ability to pass background check and drug screen as required by the facility

Education

• Graduate of an accredited practical nursing program or equivalent certification

Knowledge & Skills

• Solid medication administration and documentation practices
• Patient assessment and basic wound care knowledge
• Strong communication and teamwork skills with patients and staff
• Time management and ability to prioritize in a busy clinical environment
• Basic familiarity with electronic charting or willingness to learn quickly
